To our students

Have you changed your address or just need to update your student details or you are registering online as a student. You can now do it by simply fill in the form below and we will update your student details or register you as a student in the club. All details submitted in this form will be kept for club use only and will not be sold or provide to anyone else.

If you are registering with our club for the first time you will need to print the following forms:

The Aikido Yuishinkai Release and Indeminty Form (Retain the first page of this form as it is the Student Code of Conduct but return the 2nd page, the Release and Indeminty form to the instructor at your next lesson)

The Aikido Yuishinkai Grading Syllabus (You may just choose to save a copy of the document or print it and start a training folder)

*Club forms.
I have filled in the club Release and Indemnity Agreement (And in the case of students registering for the first time. Iwill return it to the instructor next training session)
I do have a copy of the and Student Code of Conduct
I understand that I will be given a copy of the Club Rules when I return the Release and Indemity form
I have veiwed a copy of the clubs Risk Mamagement Policy and understand that I will be expected to follow the policy
I need the sign in detail for the members area (Please note these details will only be provided to you at training)
I have completed all required club forms in the past and just wish to update my student details
